DIAM: COVID-19 Daily Screening Survey required when going to campus

All associates are required to complete the COVID-19 Daily Screening Survey prior to every DIAM Southfield campus visit. 

Completing the survey helps protect the health and safety of those who need to work on campus. Associates on campus are counting on you to complete the survey honestly and consistently every time you come on-site.

How to Access the Survey

• Survey Link: densomedia-na.com/coronavirus/survey/ (Note: if you are using the Outlook mobile app, you will need to download the Edge browser from the Company Portal in order to view this link). If you go to campus regularly – bookmark this link!
• DENSO Mobile: Go to densomobile.com and create your account or login. Once you’re on DENSO Mobile, click on the scrolling graphic at the top of the app labeled “COVID-19 Screening Survey”.
• DENSO Online: Go to densoonline.com and click on the “Coronavirus” section (password: denso2020). Find the survey under “Key Documents & Links”.

How to Complete the Survey

• You must take the survey every day that you visit DIAM Southfield campus.
• You must complete the survey prior to coming to campus.
• After you take the survey, you will be redirected to a confirmation screen where you’ll see a completion date and approval or denial to enter campus – you must take a screenshot of this screen and show this confirmation screen to the Security Guard upon your arrival.
• If you do not show a confirmation screen with the current date, you will be asked to pull aside and take the survey, and/or you will be refused entry.

Importance of the Survey

• The survey includes 5 questions that will help us take precautionary measures to protect you and everyone in the facility from spread and/or exposure to COVID-19.
• Based on your responses to survey questions, you will be informed if you should report to work.
• It’s critical that you respond to survey questions honestly to protect yourself, and the health and safety of others.
