DIAM – COVID Exposure Assessment Form
To protect yourself and others from the risk of COVID-19, associates are required to report their positive COVID diagnosis or high suspicion of COVID exposure and symptoms immediately to the COVID Task Force. All reports are confidential.
Complete the COVID Exposure Form
To make it easier for associates to report their confirmed positive COVID result and/or high suspicion of COVID exposure and symptoms, complete the DIAM – COVID Exposure Assessment Form and send to covid@na.denso.com. Access the form anytime here on DENSO Online, or on DENSO Portal: Search “covid exposure”.
Notify Your Manager
Associates are responsible for notifying their manager of their positive diagnosis and/or if they may have been exposed to COVID immediately. This allows management and the COVID task force to take quick action to notify individuals who may have been in contact and disinfect common areas.
Managers are required to keep associate name and information confidential when notifying others of potential COVID exposure.
