DIAM: Daily COVID-19 Screening Survey

To go to DIAM Southfield campus, associates must complete the mobile DENSO COVID-19 Daily Screening Survey. This must be completed prior to every campus visit.

• The survey includes 5 questions on your general information, travel and symptoms, designed as an early indicator to keep you and your colleagues safe. Please read the questions on the survey carefully, respond honestly, and elect to stay home if you are symptomatic.
• For questions related to international travel: Anyone who lives in Canada traveling to work at DIAM is allowed, as specified in the Governor’s order exemptions.

• To ensure speedy entry, complete the survey on your mobile device prior to coming to campus – you will be required to show a screenshot confirmation that you completed the survey on your mobile device to security personnel.
• If you do not have a smartphone, or forget your mobile device, you will be required to answer the questions verbally on-site.
