DENSO COVID-19 Screening Survey

In addition to our DENSO NA Covid-19 Safety Protocols, DENSO is still requiring a daily Health Screening Survey. This rule requires that DENSO shall maintain a record of health screening for each employee or contractor entering the workplace. Please read the updated health screening survey carefully and answer each question honestly to the best of your ability. When reviewing questions, evaluate for symptoms NOT caused by another health condition.

- Please check to confirm you have completed the required COVID-19 Return to Work training.
1. Have you been in close contact (less than 6 feet for greater than 15 minutes) with a confirmed case of Covid-19 within the last 5 days (If you are fully Vaccinated, Recovered from Covid-19 within the last 90 days or Cleared by medical professional answer NO)?
2. Are you experiencing a cough, shortness of breath, sore throat or fever in the last 48 hours?
3. Have you had a loss of taste or smell?
4. Have you had vomiting or diarrhea in the last 24 hours?
If you are able to answer "YES" to one or more of the above questions, select YES.

If you are able to answer "No" to all the questions, select NO.

DENSO will use the information you provide for the purpose of providing you access to the facility.
